Understanding Your Explanation of Benefits (EOB) and Lab Bill

At Proteus Genomics Laboratories, we believe in transparency and helping our patients understand every aspect of their laboratory services — including billing and insurance. Below are answers to some of the most common questions we receive.

An EOB is not a bill. It’s a statement sent by your insurance company after your lab claim is processed. It outlines:

  • The services that were billed to your insurance

  • The amount your insurance paid

  • The amount (if any) you may owe to the lab

Your EOB helps you understand how your insurance benefits were applied for your lab testing.

It’s common to receive both:

  • The EOB comes from your insurance company (for your records only).

  • The bill comes from Proteus Genomics Laboratories (for any remaining balance your insurance didn’t cover).

Receiving both doesn’t mean you’re being billed twice — they serve different purposes.

Your insurance company automatically generates and mails or posts the EOB to your online portal after a claim is processed. Proteus Genomics Labs does not issue or control your EOB.

You may receive a bill if:

  • Your insurance did not cover the full amount

  • You have a deductible, coinsurance, or copay that applies

  • Your insurance denied coverage for a portion of the test

If you have questions about coverage, please contact your insurance provider directly.

Laboratory testing is billed independently from your physician’s visit.

Your doctor’s office orders the test, but Proteus Genomics Laboratories performs the analysis and submits a separate claim to your insurance.
